Client database on access

Description

I would like an access database which will hold customer details (name, telephone number, email address).
Medical details (comprising of illnesses, medication, injuries, consent from the doctors, upload function where I can attach a PAR-Q and consultation paperwork).
Treatment details (this will have numerous links to one client as I will be providing 1 massage per month. - details will be date and time of session, massage booked (dropdown with all massages I offer). Palpation and observations (open text), Areas treated (open text), invoice, next steps (check boxes) - booked for next treatment; referred out (this will need to populate a letter with all the treatment, medical information and client details.
