Woocommerce shipping setup

Description

I've attached a screenshot of shipping options that we need to setup within Woocommerce. I'm sure there is a plugin that can handle this - but could do with someone who has handled this previously to jump on as an extra pair of hands. If we need to buy a plugin that you know does this so you can set it up please let us know and we'll get that sorted so you can setup the rest.
